Arasys systems are can be serviced ONLY at an Arasys Perfector LLC authorized diagnostic and/or repair center. Service and/or repair by individual or company other than an Arasys Perfector LLC authorized diagnostic and/or repair center is strictly forbidden and will void any warranty that may be in place.
A loaner will be shipped using two day delivery service from the Honolulu Corporate Headquarters upon receipt and acceptance by Arasys Perfector LLC of the complete information requested in this form and credit card authorization as outlined.
Repair/Loaner Terms and Conditions:
Please be sure to follow the instructions below exactly to avoid any delay with the shipment of a loaner system.
The faulty Arasys must be shipped to a designated authorized diagnostic and/or repair center within two (2) business days after you receive the loaner Arasys (see Shipping Instructions and Download FEDEX Paperwork).
The Arasys Fault Diagnosis Checklist (see below) must be filled in on the internet, printed and shipped in the same box with the faulty Arasys to the designated authorized diagnostic and/or repair center.
Once this Return Goods Authorization form is received and accepted, your Customer Service representative will designate the appropriate authorized center to ship your unit for service.
The faulty Arasys must be packaged in the original box prior to shipping it to the diagnostic and/or repair center.
A deposit of $2280.00 must be processed prior to shipping the loaner. The deposit of $2280.00 is fully refundable provided that: a) the Customer ships the faulty Arasys within two business days of their receipt of the loaner Arasys (verified by the shipment tracking number) to the designated authorized diagnostic and/or repair center; b) the Customer ships the loaner Arasys to the Honolulu Corporate office within two business days of their receipt of their repaired Arasys.
Any Customer who fails ship the faulty Arasys to the designated authorized diagnostic and/or repair center within two business days after their receipt of the loaner Arasys from the Honolulu Corporate office, will be charged $500.00 per day until this loaner is received and accepted at the Honolulu Corporate office.
Once the repaired unit is received by you, the loaner Arasys must be packaged in its original box and promptly shipped to the Honolulu Corporate office.
The loaner Arasys must be returned to the Honolulu Corporate office within two (2) business days after you receive your repaired Arasys.
Any Customer who fails to return the loaner Arasys within two business days after their receipt of their repaired Arasys to the Honolulu Corporate office will be charged $500.00 per day from the time they received their repaired Arasys until this loaner is received by Corporate.
Authorization: I agree with the terms and conditions regarding the Arasys Loaner. I authorize Arasys Arasys, Inc. to charge my credit card the deposit amount of $2280.00 prior to receiving a loaner Arasys.
Warranty Repair: Please refer to the terms and conditions of your Purchase Order agreement for your New Arasys, Upgrade and Extended warranty coverage. Minimum Diagnostic/Repair Charges: If the Customer's Arasys is found to be fully functional and no fault is found by the designated authorized diagnostic and/or repair center, then I forfeit my $2280.00 deposit. In addition I understand that my credit card will be charged and additional $380.00 for diagnostic services. Additional charges may apply for shipping, handling and insurance. Late Return Penalty: I understand tha't my credit card will be charged $500.00 per day if I fail to comply with the time restraints in sending my faulty Arasys system to the Honolulu Corporate Headquarters for repair or for not returning my loaner Arasys as stated and agreed above.
